If you are new to the practice, please note we have a paperless office. We use an Electronic Health Record (EHR) system called Simple Practice. Simple Practice keeps all data secure and information private with bank-level encryption that is HIPPA compliant.
Please note that all clients are required to complete a New Client Intake Form, Authorization and Consent Forms, Provide an Emergency Contact and have a credit card on file for the purpose of fees related to copays, coinsurance, late cancellations or no shows. All forms must be completed prior to your appointment to avoid cancellation and all payments are within 10 days of receiving the invoice for payment.
Current clients can make payments electronically via the Simple Practice Client Portal, when seeing their therapist in the office or click here and make a payment now from the website.
Clients also have access to the therapists’ calendar showing openings for in-office and Telehealth appointments. This process bypasses the hassle typical of making appointments on the phone or even in person. Clients can schedule appointments up to 3 months in advance.
